Health Permit Procedures

Chilean Sea Bass Plate

The following are guidelines for Show Managers whose exhibitions provide food samples and wish to have one permit cover all locations:

1) The Licensee must provide, on their letterhead, a letter to the Health Department stating the following:

Attn: Temporary Permits

2) The Licensee must provide the Health Department with a floor plan showing the names and locations of each food booth covered by the permit. Each booth must complete a Food Booth Menu Report Sheet, and these sheets must accompany the floor plan. (See attached Food Booth Menu Report)

3) The Lessee must contact the Health Department to set up a meeting to present the above items and complete the final application for the Health Permit. This meeting should be scheduled at least 21 days prior to the show.
