An officer of the Licensee Corporation, Company, etc must sign this letter.

2)

The Licensee must provide the Health Department with a floor plan showing the names and locations of each food booth covered by the permit.

Each booth must complete a Food Booth Menu Report Sheet and these sheets must accompany the floor plan. (See attached Food Booth Menu Report.)

3)

The Lessee must contact the Health Department to set up a meeting to present the above items and complete the final application for the Health Permit. This meeting should be scheduled at least 21 days prior to the show.
