| The following are guidelines for
Show Managers whose exhibitions provide food samples
and wish to have one permit cover all locations:

| 1) |
The Licensee must provide, on their letterhead,
a letter to the Health Department stating the
following:
Consumer Health Services Division
Food Inspection Bureau
8000 N. Stadium Drive, 2nd Floor
Houston, Texas 77054
Attn: Temporary Permits
|
“We (Licensee Name) submit
by this letter verifying we are assuming all legal
liabilities which may occur from any food and
beverage sampling services performed at the George
R. Brown Convention Center, Houston, Texas, during
the (show name and dates).”
An officer of the Licensee Corporation, Company,
etc must sign this letter.

| 2) |
The Licensee must provide the Health Department
with a floor plan showing the names and locations
of each food booth covered by the permit.

Each booth must complete a Food
Booth Menu Report Sheet and these sheets must
accompany the floor plan. (See attached Food Booth
Menu Report.)

| 3) |
The Lessee must contact the Health Department
to set up a meeting to present the above items
and complete the final application for the Health
Permit. This meeting should be scheduled at least
21 days prior to the show.

When the permit is issued, it must
be displayed and available at the show desk. For
